What ADHD Reveals About School, Status, and the Attention Economy

Cap, a high school student from an affluent East Coast suburb, hated taking Ritalin. “Honestly, I pretty much hated taking it,” he told researcher Paul Tough. But he took it anyway—for SAT prep, for baseball practice, for getting through school.
The medication worked, in a way. “I was so focused,” he said about batting practice on Ritalin. “My contact rate was higher. I could see the ball better.” For studying, too: “I used to hate doing the SAT reading. But if I took the medication, I could read through it all and, like, comprehend it really well. I would actually enjoy reading it.”
But there was a cost. “When I’m on the medication…,” Cap explained. “it feels like you’re a horse with blinders on. You’re just focused on your goal. There’s nothing else going on in your head.”
When the medication wore off, he felt awful. By senior year, he was using Ritalin strategically—more often for baseball games than for studying. And during summers, when he wasn’t in school? He rarely took it at all.
His teammate John had a similar experience. Adderall helped him get through English class, which he never liked much. “It would make it so that if I tried to pay attention, I would be able,” he said. “It would still be very boring, but I was able to finish books and pay attention to what was happening.”
But the medication flattened him. “Around my friends, I’m usually the most social, but when I’m on it, it feels like my spark is kind of gone. I laugh a lot less. I can’t think of anything to say. Life is just less fun.”
These students’ experiences capture what I’ll call the attention paradox: their attention problems aren’t constant or global. They’re situational, contextual, and selectively real. The question isn’t whether their struggles are genuine—they clearly are. The question is what it means that attention “deficits” so often come and go depending on the task, the setting, and what’s at stake.
Paul Tough’s recent New York Times Magazine investigation into ADHD surfaces a truth that clinicians acknowledge privately but rarely state publicly: the condition that now affects more than seven million American children—and whose treatment has become a multi-billion-dollar pharmaceutical market—is far messier, more contextual, and less clearly “medical” than the dominant narrative suggests.
The diagnosis lacks a biomarker. The symptoms fluctuate dramatically across settings. The medications produce striking short-term behavioral changes but questionable long-term benefits. And most tellingly, many people diagnosed with ADHD can focus intensely—but only when the task feels meaningful, immediate, or intrinsically rewarding.
This pattern should prompt a fundamental rethinking of how we understand attention problems. Not as evidence that ADHD is fake or that struggling students are simply unfocused, but as a signal that attention is not just a cognitive capacity you either have or lack. It’s relational and situational: shaped by meaning, context, motivation, and fit between a person’s neurology and their environment.
The question isn’t whether attention problems are real. The question is what it means that they are so often selectively real.
What Tough Gets Right—And Why It Matters
Tough’s essay makes several crucial interventions that challenge the conventional wisdom about ADHD:
The diagnosis is fuzzier than we admit. Unlike diabetes—the comparison ADHD advocates often invoke—there is no blood test, brain scan, or genetic marker that reliably identifies who has ADHD and who doesn’t. The diagnostic criteria in the DSM require subjective judgments about whether a child displays “six or more” symptoms of inattention or hyperactivity “often” enough and with “sufficient severity.” These thresholds are practical conventions, not natural boundaries carved into brain biology.
ADHD is heterogeneous, not uniform. The diagnosis encompasses wildly different presentations: children who can’t stop talking and children who barely speak, risk-takers and anxious perfectionists, the hyperactive and the dreamy. Some struggle primarily with focus; others with impulse control; still others with emotional regulation. Lumping them together under one label obscures more than it reveals.
Stimulants change behavior more than learning. The most troubling finding Tough showcases comes from long-term follow-up studies showing that while medications like Ritalin and Adderall can produce dramatic improvements in classroom behavior—children sit still, complete worksheets, follow instructions—they produce minimal gains in actual academic achievement. Students on medication do more seatwork, but when tested weeks later, they’ve learned no more than students who weren’t medicated. They look more focused, but the cognitive benefits are elusive.
The advantages fade. The landmark Multimodal Treatment of Attention Deficit Hyperactivity Disorder (MTA) study showed that after 14 months, children taking stimulants had fewer symptoms than those receiving only behavioral therapy. But by 36 months, that advantage had disappeared entirely. Children in every group—including the comparison group that received no systematic treatment—displayed similar symptom levels. Meanwhile, children who took stimulants consistently ended up, on average, more than an inch shorter than their peers by early adulthood.
Context matters profoundly. Perhaps most importantly, Tough documents how ADHD symptoms fluctuate across environments. Students whose attention collapses in English class can focus for hours on video games, building projects, or athletic practice. Adults who struggled in school thrive once they find work that matches their temperament—hands-on trades, fast-paced environments, creative fields. Their “disorder” didn’t disappear; the mismatch between their attentional style and their environment did.
These findings don’t prove that medication never helps. They prove something more nuanced: that the brain disorder doesn’t capture what’s actually happening.
The Missing Idea
The conventional medical model treats attention as a resource—something you have in limited supply, like willpower or glucose. People with ADHD, in this view, have depleted tanks. They can’t sustain focus because they lack the necessary cognitive capacity.
But the attention paradox suggests a different model: attention is better understood as a relationship between a person and a task, shaped by meaning, structure, reward, and context.
We attend to what feels consequential. We attend to what we understand. We attend to what offers feedback and progression. We attend to what connects to our identity, interests, or social world. When those conditions are met, even people diagnosed with severe ADHD can demonstrate remarkable focus.
The brain’s dopamine systems—which regulate attention, motivation, and reward—respond to meaningful engagement, not just effort. When a task feels pointless, abstract, or disconnected from anything that matters, the neural systems that sustain attention literally disengage. This is true for everyone, but people with ADHD experience the disengagement more acutely.
Consider three dimensions of attention:
Capacity: Can you sustain focus at all? Some people genuinely struggle with basic attentional endurance—they fatigue quickly even when motivated. This is closest to the traditional medical model and probably has the strongest biological basis.
Control: Can you direct focus where you choose, even when the task is boring or difficult? This is executive function—the ability to override immediate impulses in service of longer-term goals. It develops through childhood and can be taught, but it’s also clearly uneven across individuals.
Context sensitivity: What conditions make focus possible? This is where environment, meaning, structure, and fit come in. A student who can’t focus on textbook reading but can focus on documentary filmmaking doesn’t lack attention—they have attention that responds to different conditions than those school typically provides.
The traditional model treats all three as one thing: “attention deficit.” But they’re distinct, they have different causes, and they respond to different interventions.
Saying “they can focus on video games” doesn’t refute an ADHD diagnosis. It diagnoses what kind of problem they have: not a global incapacity for focus, but a narrower profile where attention depends heavily on task design, meaning, and feedback.
The Severity Spectrum
One of Tough’s most important contributions is forcing us to distinguish between disorder and ordinary childhood. Not every fidgety, forgetful, impulsive child has a brain disorder requiring medication. But some children do face serious, persistent impairments that disrupt learning, relationships, and safety across multiple contexts.
We need clearer categories:
1. Developmental immaturity. Young children, especially boys, naturally struggle with impulse control, sustained attention, and sitting still. Many outgrow these difficulties without intervention. Diagnosing seven-year-olds with ADHD when they’re simply behaving like seven-year-olds medicalizes normal development.
2. Situational dysfunction. Some children function well in most settings but falter in specific ones—typically those requiring sustained attention to abstract, low-interest material. This is a real problem, but it’s as much about task-environment mismatch as about neurological deficit. The solution may be environmental redesign, skill-building, or strategic use of medication—not necessarily lifelong pharmaceutical treatment.
3. Mild cross-context impairment. These children struggle noticeably at school and home but can function with support, structure, and accommodations. They benefit from executive function coaching, behavioral strategies, and sometimes medication. But their difficulties are manageable.
4. Severe, persistent impairment. These children—perhaps a third of those diagnosed—struggle across all settings: school, home, peers, and extracurriculars. They face chronic disorganization, relationship conflicts, academic failure, and sometimes safety risks (impulsive behavior leading to injuries, accidents, or dangerous decisions). Many have comorbid conditions: learning disorders, anxiety, depression, trauma histories. This group needs comprehensive, sustained intervention—often including medication, but rarely medication alone.
5. High-risk profiles. As researcher Joel Nigg has identified, some children with ADHD symptoms also exhibit intense anger and emotional dysregulation. This subgroup is at much higher risk for school dropout, criminal behavior, substance abuse, and serious injury or early death. They need early, intensive, multimodal treatment.
The current system lumps all five categories together, applies the same label, and often defaults to the same pharmaceutical intervention. That’s not precision medicine. That’s diagnostic laziness.
Diagnosis should track impairment and persistence, not teacher annoyance, parental anxiety, or schools’ desire for classroom compliance.
The Medicalization Turn—And the Historical and Cultural Dimension
ADHD sits at the intersection of several converging forces that help explain why diagnoses have tripled since the 1990s:
A schooling system increasingly intolerant of variability: Schools have always required attention and compliance, but contemporary schools demand longer periods of sedentary focus on abstract tasks, starting earlier in childhood. Kindergarten now looks like third grade used to. Recess has been cut. Movement breaks have disappeared. Children who would have been merely “rambunctious” a generation ago now get flagged as disordered.
The college admissions arms race: Middle-class families face intense pressure to optimize children’s academic performance. ADHD diagnosis—and the medication that comes with it—becomes a tool for competitive advantage. Some parents seek diagnosis specifically to obtain extended time on standardized tests. Others want medication to help their children handle overloaded schedules.
The therapeutic turn: Contemporary culture increasingly interprets struggle as pathology requiring professional intervention. What previous generations might have called “laziness,” “immaturity,” or “lack of discipline” now gets diagnosed as ADHD, executive function disorder, or oppositional defiant disorder. This isn’t entirely wrong—some children labeled “lazy” were actually struggling with genuine impairments—but it does mean the diagnostic net has widened dramatically.
A radically altered attention ecology: Children today grow up immersed in screens delivering variable-ratio reward schedules optimized to capture attention. Video games, social media, YouTube, and TikTok provide constant novelty, immediate feedback, and unpredictable rewards—precisely the conditions that make sustained focus on slower, less immediately rewarding tasks (like reading textbooks) feel unbearable by comparison. It’s not clear whether this creates ADHD or simply makes pre-existing attentional vulnerabilities more visible and impairing.
Institutions under strain: Schools and families have less capacity than they once did to provide patient scaffolding, one-on-one attention, and individualized support. Class sizes have grown. Teachers are overwhelmed. Parents work longer hours. In this environment, medication becomes the most scalable intervention—faster and cheaper than hiring more teachers, reducing class sizes, or redesigning curriculum.
None of this means ADHD is “caused by culture.” It means culture shapes what counts as impairment, where it shows up, and which solutions become default.
Beyond the Binary: A Layered Approach to Treatment
The current system presents families with a false choice: medication or nothing. Accept the diagnosis and the prescription, or struggle alone. This is deeply inadequate.
A more honest, effective approach would be layered:
Differential diagnosis: Before assuming ADHD, rule out other causes of attention and behavior problems: sleep disorders (extraordinarily common and often overlooked), anxiety (which produces distractibility that mimics ADHD), depression (which saps motivation and focus), trauma (which disrupts emotional regulation and executive function), learning disorders (struggling students often look inattentive when they’re actually confused), sensory processing issues, medication side effects, substance use (in adolescents), and chronic stress.
Skill-building: Many children can learn executive function skills: breaking tasks into smaller steps, externalizing memory (using lists, reminders, calendars), creating routines, time-boxing work sessions, and reducing friction to getting started. These are teachable. They don’t cure ADHD, but they make it more manageable.
Environment engineering: Change the task before changing the brain. Shorter work cycles with built-in breaks. More frequent feedback. Movement integrated into learning. Choice over assignments. Project-based work rather than abstract exercises. Hands-on activities. Making expectations visible and concrete. Reducing the cognitive load required to start tasks. These accommodations help everyone but are especially crucial for ADHD students.
Relationship supports: ADHD strains parent-child relationships (constant nagging about homework, organization, and behavior) and teacher-student relationships (frustration, discipline, failure). Reducing conflict, rebuilding trust, and addressing shame are often as important as addressing symptoms. Parent behavioral training, family therapy, and teacher coaching can all help.
Medication—when appropriate: Stimulants can be valuable tools. For some children, they make the difference between functioning and failing. But they should be prescribed with honest trade-offs acknowledged. They work best for short-term behavioral control, not long-term learning gains. Benefits often fade over time. Side effects can include appetite suppression, sleep disruption, mood changes, growth suppression, and in some cases, increased risk of psychosis. They don’t teach skills, provide meaning, or fix environments. Many children hate how they feel on medication
The key insight is obvious: Medication may widen the “window of self-control,” but it can’t supply meaning, purpose, structure, or a workable life design.
What the Attention Paradox Reveals About Schools
The most subversive implication of Tough’s reporting is what it suggests about schools themselves.
If attention reliably returns when work is meaningful, structured, socially supported, and intrinsically engaging, then perhaps the problem isn’t primarily in children’s brains. Perhaps it’s in environments that demand prolonged compliance with tasks that even adults would find mind-numbing.
Schools were designed for a different era: preparing factory workers who could tolerate repetitive tasks, follow orders, and sustain attention through boredom. We’ve grafted 21st-century content onto this 19th-century structure and then diagnosed children as disordered when they can’t tolerate it.
The students who do well in traditional school aren’t necessarily smarter or more capable. They’re the ones whose attentional profiles happen to match what school demands: tolerance for abstraction, delayed gratification, extrinsic motivation (grades, college admissions), and low need for movement, novelty, or immediate feedback.
Students whose profiles don’t match—who need hands-on work, who crave novelty, who struggle with tasks they find meaningless, who learn better through doing than through reading—get labeled deficient rather than different.
This doesn’t mean we should reduce reading, writing, and math. It means we should ask: What forms of attention are we actually trying to cultivate? And are the structures we’ve built the best ways to develop them?
Rethinking Identity: From Deficit to Difference
One of the most fraught questions Tough explores is how diagnosis affects identity. For some children and families, ADHD diagnosis brings relief—an explanation for years of struggle, validation that the child isn’t “bad” or “lazy,” and access to support and accommodations. The medical framing reduces moral blame.
But for others, being told “you have a brain disorder” becomes a life sentence. It suggests something fundamentally broken that can’t be fixed—only managed with daily medication. It can reduce expectations (”he has ADHD, so we can’t expect him to...”) and create learned helplessness (”I can’t do this because of my ADHD”).
The alternative framing Tough points toward is more hopeful: You have a particular attentional profile—one that’s more context-sensitive than average, that struggles with certain kinds of demands but excels in others, and that requires you to be strategic about environment and support.
This isn’t denying difficulty. It’s refusing to collapse difference into deficit.
The most revealing evidence comes from the MTA study’s long-term follow-ups. When researchers interviewed subjects in their mid-twenties, many reported that their ADHD symptoms had essentially disappeared—not because their brains changed, but because they found environments that fit. They chose jobs that were physically active, or cognitively demanding in ways they found engaging, or entrepreneurial, or creative. They discovered “niches” where their attentional style was an asset rather than a liability.
A young film student told researchers: “Originally, when I was first diagnosed with it, it was explained to me as attention deficit, just a lack of attention. An ability not to have an attention span for very long. But I can have an attention span for extremely long for the things that I care about.”
He reflected on his school struggles: “Public education, you’re forced into it. Maybe that’s why I didn’t pay attention that much. But now I’m in college, in a subject that I want to be a part of, so me having a lack of attention, it hardly ever happens anymore, because I’m not usually where I don’t want to be.”
A hairstylist said her inability to concentrate in school vanished when she began studying hair. “If you sit up there and give me a lecture on a haircut, I will remember everything you said, word for word. Stuff that I’m into, I am so immersed in it. But in school, it was awful.”
An auto technician training said: “It’s just that I had to figure out what I wanted to do. I want to work with cars. I don’t get bored doing that.” He went further: “If people with A.D.H.D. are directed into areas where their strengths and interests lie, I’m pretty sure that they can naturally just go about dealing with it, instead of having to give people medications.”
These aren’t people whose ADHD was cured. They’re people who stopped being in environments that demanded the kind of sustained attention they struggle to provide.
The Problem of Inescapable Environments
The “find your niche” solution is humane and often true—especially for adults with autonomy over their lives. But it has a dark side: many children can’t escape bad environments.
A child in a chaotic household with stressed, overwhelmed parents can’t redesign their home life. A student in an overcrowded school with burned-out teachers and rigid curriculum can’t opt into better pedagogy. A teenager from a low-income family can’t afford private school, homeschooling, or alternative programs.
For these children, saying “the problem is environmental mismatch” is true but unhelpful. The environment is bad and unchangeable on the timeline of their childhood. In these cases, medication may be the most practical tool available—not because the child is defective, but because the setting is.
This is the tragic compromise at the heart of ADHD treatment: We know schools could be better designed. We know families need more support. We know that smaller classes, more movement, more choice, more hands-on work, and more meaningful tasks would help. But those changes require political will, sustained funding, and cultural transformation that aren’t coming.
So we give children pills instead.
That’s not ideal. But it’s also not nothing. For many children, medication makes a non-negotiable environment tolerable enough to get through. That’s worth something—as long as we’re honest about what we’re doing and why.
Toward a More Honest Model
The synthesis Tough points toward—and that evidence supports—is this: ADHD names a cluster of attention-regulation difficulties that become disabling in certain environments. Those difficulties have biological components (genetic vulnerabilities, neurodevelopmental variations, dopamine system differences), psychological components (motivation, meaning, self-concept), and environmental components (task design, social support, structure, fit).
The severity ranges from mild to severe. The causes are multiple. The trajectory fluctuates. And the best responses are plural: environmental redesign when possible, skills training, relationship repair, addressing comorbidities, and—when appropriate—medication prescribed with honest information about benefits, limits, and trade-offs.
This model abandons the clean either/or that diagnostic battles usually demand: either ADHD is a real brain disorder requiring medical treatment, or it’s a cultural invention that medicalizes normal childhood. Both extremes are wrong.
ADHD is real suffering—but suffering produced by complex interactions between biology and context, not by a single defective brain module. Treatment should be individualized, multimodal, and revisable as contexts and capacities change.
What We Owe Children
The real scandal isn’t that children fidget, forget things, or struggle to focus on boring tasks. The scandal is that we’ve built institutions that require prolonged compliance from bodies and minds built for movement, novelty, and meaning—and then we diagnose children as disordered when they can’t meet demands that adults would find intolerable.
That doesn’t mean every restless child is fine and schools are the only problem. Some children do have profound, persistent impairments that require serious intervention. But it does mean we need to ask harder questions:
▪ What kinds of attention do we actually want to cultivate, and why?
▪ Are the structures we’ve built the best ways to develop those capacities?
▪ When should we change children to fit environments, and when should we change environments to fit children?
▪ How do we distinguish genuine disorder from mismatch, and both from normal developmental variability?
▪ What do we owe children when environments can’t be fixed on the timeline of their lives?
Tough’s essay doesn’t answer all these questions. But it does something more important: it makes the questions unavoidable. And it suggests that the answer isn’t simply more diagnosis and more medication, but more humility about what we know, more honesty about trade-offs, and more willingness to redesign environments rather than just medicating the children struggling within them.
Cap and John—the high school students whose experiences opened this essay—don’t have broken brains. They have brains optimized for certain kinds of engagement and challenge, being asked to spend six hours a day in environments designed for a different kind of mind.
We can give them Adderall to get through, and maybe we should. It’s a tool that helps them navigate non-negotiable environments. But we should also remember John’s insight: “If I don’t have to do any work, then I’m just a completely regular person.”
That observation should haunt us. It suggests that the “disorder” isn’t entirely in John, but in the perpetual mismatch between who he is and what we’re asking him to do.
The question the attention paradox poses is whether we have the courage to redesign education for students like Cap and John—or whether we’ll continue medicating them until they can tolerate what we’ve built.
It’s a question we can no longer avoid.
