From that vantage point, the recent transformations in medical thought are striking—not simply for their scientific significance, but for the cultural worlds they created and the moral narratives they invited.
These shifts were never confined to laboratories or academic journals. They altered patient care, reoriented public health, transformed how ordinary people imagine illness, and redefined what it means to be healthy. More profoundly, they changed how we understand the relationship between body and world—between biology and experience, self and environment.
If the 19th century belonged to microbes and sanitation, the 20th and early 21st centuries have been marked by a succession of new master concepts: the immune system, stress, inflammation, the genome, epigenetics, and the microbiome. Each framed illness in a new way and offered a fresh vocabulary for thinking about human vulnerability and resilience.

It implied that “stronger immunity” is always better, overlooking the dangers of overactive immune responses. And it individualized health, treating each patient as an isolated fortress rather than a participant in broader ecological and social systems.
The immune-system paradigm was enormously productive—but like all metaphors, it both illuminated and obscured. Its legacy continues to shape how we think, feel, and talk about illness today.
The Stress Revolution and the Embodiment of Modern Life
If the immune system defined mid-20th-century medicine, stress became the dominant metaphor of late-20th-century life. Stress offered something the immune paradigm could not: a way to explain how the very conditions of modernity—its pace, pressures, and insecurities—could make people sick.

Gene therapy would cure inherited disorders at their roots. “Personalized medicine,” tailored to each person’s DNA, would revolutionize treatment. Cancer itself would become manageable once its genetic drivers were mapped and targeted.
Reality proved far more complicated than the rhetoric suggested.
The Human Genome Project showed that very few diseases are linked to single genes. Most common conditions are polygenic (shaped by many genes, each with tiny effects) and multifactorial (dependent on complex interactions between genes and environment). The boundary between “self” and “other,” “human” and “non-human,” dissolves. You are not a human who hosts bacteria—you are a consortium of human and microbial life.
This reconceptualization resonates with ecological thought, posthumanist philosophy, Indigenous knowledge traditions, and feminist theories of relational selfhood. We now live inside a medical culture shaped by all of these frameworks at once. These paradigms do not cancel one another; they overlap, offering different angles on the same complex reality.
Medicine today is more integrative, systemic, and complexity-aware than ever. It sees health as emerging from interactions across multiple scales—molecular, cellular, psychological, social, environmental. The boundaries between body and world, biology and experience, individual and context have become porous.
This sophistication represents real progress. But it brings new challenges. Complexity can overwhelm both clinicians and patients. And despite widespread embrace of “biopsychosocial” language, the practice of medicine still often defaults to narrow specialization, brief visits, and treatment plans that fit reimbursement models rather than human lives.
These conceptual frameworks also carry risks. They can:

The body is biological, but illness is always more than biology. It is shaped by place, class, race, exposure, stress, support, and possibility.
Medicine must therefore grow not only scientifically but also philosophically and humanistically. It must hold the molecular and the social together; see patients as bodies and selves; communicate clearly while respecting complexity; and remain aware that every scientific framework is also a metaphor, carrying cultural assumptions about what bodies are and what they should be.
The dominant metaphors of modern medicine—defense, stress, inflammation, code, inscription, ecosystem—are not just technical descriptors. They shape how we imagine health, responsibility, vulnerability, interdependence, and what we owe one another.
Understanding what these metaphors reveal—and what they obscure—is as essential to good medicine as understanding the mechanisms themselves.
New frameworks will emerge, each promising clarity and each creating its own blind spots. The history of medicine counsels humility: every paradigm eventually shows its limits. But it also offers hope: each has broadened our understanding and, in its own way, reduced suffering.
We can already see the next paradigm shift unfolding. Anti-obesity drugs—especially GLP-1 agonists like Ozempic and Wegovy—are transforming how we think about illness, agency, and medical intervention. Long framed as a matter of willpower and personal responsibility, obesity is now understood as a condition governed by complex physiological systems: appetite regulation, reward pathways, gut hormones, metabolism. A weekly injection that suppresses appetite and alters digestion disrupts the old moral narrative and reframes obesity as a medical, not moral, problem.
Culturally, these drugs challenge longstanding beliefs about discipline and blame. Economically, they threaten entire industries built on dieting, wellness, and bariatric surgery while creating new markets organized around pharmaceutical management of appetite and metabolism. And ethically, they force us to confront questions about body image, access, equity, and the medicalization of difference.
The lesson is the same one history keeps teaching: scientific advances don’t just change treatments; they reshape the stories we tell about health, responsibility, and the meaning of illness.
Our task, going forward, is to welcome these advances while keeping our metaphors flexible, our frameworks provisional, and our imagination open. No single model—no matter how powerful—can capture the full reality of the human body or the lived experience of suffering. The more consciously we hold that truth, the better placed we are to build a medicine that is not only more effective, but more humane.
